Our presenters will discuss how newly approved personal care, homemaker and respite benefits are driving health equity and impacting people’s lives via the lens of a health plan and a provider.

Lisa Chan-Sawin is CEO and founder of Transform Health, LLC, and has more than 15 years of experience in policy analysis, program development, strategic planning, and implementation of large systems transformation projects, with expertise in Medicaid waivers. Her prior experience includes serving as a chief innovation officer for a delivery system reform incentive payment program at a national health policy consulting firm, as well as the California Coverage and Health Initiatives organization, the California Assembly Health Committee and California Senate Health Committee.
April Stewart serves as the Vice President of Access and Innovation at 24 Hour Home Care and has been with the company nearly 10 years. In her role, she oversees healthcare program development, program expansion and healthcare industry advocacy for Medicaid waiver service lines. She brings 10 years of experience in home- and community-based services, operations, sales, healthcare advocacy, program development and contracting.
